Professional Services Group is contracted to provide case management services through the Dane County Department of Human Services. The Children’s Long-Term Support (CLTS) Waiver Program is a Medicaid-funded, home and community-based support program designed for children and young adults under the age of 22 who have significant developmental, physical, or emotional disabilities. The goal of the program is to enable participants to live independently at home and in the community rather than in institutional settings. CLTS and Children’s Community Options Program (CCOP) funding can be used to support a range of services based on an assessment of the needs of the children and their family. A deciding together approach is utilized which is a collaborative process where families and support teams work in partnership to explore options, share information, and make decisions that best support the child’s needs and family’s goals.
