Job Description

Due to expansion, we are looking to fill full time positions with our Families First Program. This program provides case management services to children with severe emotional disabilities (SED) and their families who are at-risk of psychiatric inpatient hospitalization or other out-of-home placements.

Responsibilities include: interview and evaluate clients and families to complete intake and assessment process; develop a family support team comprised of formal and informal supports; attend client/family court proceedings; research community resources to assist the client and family in meeting their goals; participate in weekly program staffings and present assigned cases; and work collaboratively with other providers, school personnel, mental health professionals, DCFS workers and court officials.

Hours of this position are Monday through Friday, between the business hours of 8am and 6pm.

Must be able to enter client/family contacts into identified databases and complete all documentation requirements.

Must possess a Bachelor's Degree in a human services field and have one year's experience working with at-risk youth and families.

Must possess a valid driver's license and proof automobile insurance. Background check and drug test required.

Strong oral and written communication skills necessary.
