Job Description

This position is Limited Term Employment (LTE) – six months. Working hours will be primarily between the hours of 8:00 am and 5:00 pm, but will also include occasional early evening hours, based on the needs of the program and its clients and number of hours will vary week to week.

Responsibilities will include providing information, outreach and assistance to clients as well as the public; collection of information to determine service eligibility and need and acceptance of referrals for services. The Social Worker will be expected to demonstrate engagement and responsiveness with clients and their support teams; assess current functioning and priorities; determine the type, times and range of services or supports required and develop a comprehensive outcome focused service plan in response to the needs that have been identified. Once a plan has been determined and implemented, worker is expected to provide, coordinate, monitor, and/or adjust services as needed, assuring their ongoing focus, effectiveness and efficiency.

Requirements

Applicants must possess a Bachelor’s Degree in Social Work, Psychology, Criminal Justice or other related field. Must be certified as a Social Worker through the State of Wisconsin and have at least one year of previous child welfare case management experience. A valid driver’s license and proof of current vehicle insurance is required. Prior to offer of employment a background check and drug test are required.
